All of forms are in PDF format that can be completed online. Regulations require that all forms must be signed before submission.
It’s easy! Go the the forms download page for the pharmacy sign-up form. Health Canada and the College of Pharmacists of Manitoba require each pharmacy to sign up with us officially and then we may begin working together as a team! Once signed up, you may request price quotes and/or order requests at any time. There is no cost to sign up. You must be signed up to obtain a price quote or place an order. You are only charged if you place an order.
Once signed up, you may request price quotes and/or order requests at any time via fax. We do not have a specific product/price list as the preparation variations are endless! Instead, you may use the price quote form to simply request the pricing of the preparation(s) that you require. Once the pharmacy/patient agrees to the cost, you may sign off at the bottom of the price quote form and fax this form back to us. Essentially, this can act as your one-time order form. After this, you may simply use the order form for items you wish to order and/or do not require a price for each time. Please note that a valid prescription is required for all compounds dispensed at your pharmacy.

Unfortunately, once the pharmacy has signed off on the order request, and the work is completed to make the preparation, the order cannot be cancelled or changed. Please make sure you are ordering the correct product, strength and amount when submitting your order.
No. Health Canada’s Policy POL-0051 (Manufacturing and Compounding Drug Products in Canada) clearly identifies activities that relate to either compounding or manufacturing. The Compounding Pharmacy of Manitoba (CPM) operates within the compounding guidelines and is licensed by the College of Pharmacists of Manitoba.
This is a dilution factor that is often meant for certain compounded orders. However, not every order is always meant to have it.
Often the new-age computer generated prescription orders automatically put the ‘1:1’ into the drug files.
As an example, if the order reads “Diclofenac 8% and Lidocaine 2% ointment (1:1)” legally this must be dispensed as Diclofenac 4% and Lidocaine 1% in order to fill the order appropriately with the indicated dilution factor.
In order to get the full-strength Diclofenac 8% and Lidocaine 2% ointment, the 1:1 should NOT be included in the instructions. This is what changes the final concentration of the preparation.
You may need to contact the prescriber to confirm the final concentration of the preparation.
